Payment Forms Accepted: Our practice gladly accepts cash, checks, money orders, and credit cards (Visa, MasterCard, Discover) for payment of copays, deductible fees and other relevant charges. Therapists are not able to provide exact change so please bring your exact copay. Checks or money orders can be made out to Southwest Florida Counseling Center or SWFCC. Copays must be paid at the beginning of session. Thank you for your prompt payments.
Insurances Accepted: Blue Cross Blue Shield, United Healthcare, Cigna, Aetna, ComPsych, Tricare, Champ VA, Beacon (formerly Value Options), Multiplan, Magellan, MH Net, Psychcare, Concordia Behavioral Health.
*Medicaid Managed Care Plans Accepted: Amerigroup, Staywell, Sunshine Health, Integral, Molina, CMS Kids, Healthy Kids
Self Pay Clients: For clients who choose to self pay, sessions are billed at a rate of $75 per hour. We also offer a sliding scale fee, based on your income. Group therapy is $30 self-pay per session.
Missed Appointment Fee: Clients who do not provide at least 24 hours advance notice of missing an appointment will be charged a $50 missed appointment fee. If you need to miss an appointment, please contact your therapist directly and leave a voicemail for them. Our therapists are paid per client session provided so providing notices allows them to schedule other people in available times. Even if you are not able to call with 24 hours notice, it is appreciated if you can call prior to your appointment to notify your therapist. Your insurance plan will not cover your missed appointment fee.
Returned Check Fee: There is a $25 fee for all returned checks.
Other Services and Fees:
- Letters: $25-50, depending on detail required
- Court Testimony Fee: $200 per hour with a 3 hour minimum. The $600 minimum needs to be paid in advance.
Holidays and Inclement Weather: Our practice does not close down for holidays or inclement weather, and therapists may individually offer services on holidays or even in inclement weather. In the case of inclement weather, please contact your therapist if you are unable to attend your session. Your therapist will contact you directly if they are unable to make it to our practice for your session.
